OFFEROR READ THIS INFORMATION FIRST BEFORE SUBMITTING AN OFFER This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0991 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-04 dated 06/02/2023 This is an unrestricted solicitation meaning all business size may respond. At the time of submission of offer and prior to award, the offeror shall be active in the System for Award Management (SAM) reflecting 621492 Kidney Dialysis Centers. The PSC Code is Q401 Medical Nursing. The VA anticipates the award of one (1) Indefinite Quantity Contract (IDC) for On-Site Acute Dialysis Services to support the Sioux Falls SD VA Health Care System located at 2501 W 22nd Street, Sioux Falls SD 57105. The period of performance is from January 1, 2024, to December 31, 2024, and includes four (4) one year option periods to be exercised at the discretion of the government. Offer Due Date: Quotes are due to the Contracting Office by November 7, 2023, at 11:00 AM Central Time. Quotes shall be emailed to [email protected] prior to the solicitation closing date and time.
