FOAM SPACERS

The Department of the Army, MICC-Ft. Drum, issued combined synopsis/solicitation W911S226U3021 for the procurement of foam spacers. These spacers must measure five inches in diameter and 3/4 inches in thickness, utilizing Class 4, Grade B, Type I polyethylene material. The products must meet all requirements of specification A-A-59136 for the specified class, grade, and type, and the agency identifies Ethafoam 220 or an equivalent as the standard. This requirement is designated as an SBA SBA set-aside associated with NAICS 325212 for Synthetic Rubber Manufacturing and PSC 8135 for PACKAGING AND PACKING BULK MATERIALS.

Required documentation includes a Safety Data Sheet and a Certificate of Conformance or Certificate of Analysis with each shipment. These certificates must contain the manufacturer name, product description, specification, lot or batch number, manufacture and expiration dates, analytical data, a statement of no change in materials or processes, and an authorized signature. Packaging labels must also display the manufacturer name, product description, lot or batch number, and manufacture and expiration dates. A Certificate of Conformance or Certificate of Analysis must be approved by the MCAAP AO before a contract award is made.
