The services required for this contract include a Base Year; Option Year One, Option Year Two, Option Year Three, and Option Year Four. The requirements for the base period may change, depending on the actual date of award.
All treatment services are to be provided by clinicians who currently hold a professional license to practice independently, without supervision, in the state of Pennsylvania.
