AMENDED SF1449: 100% Set Aside for Native American Firms IAW Buy Indian Act of 1910 This is a SOLICITATION notice 75H71223R00052 for commercial items/service prepared with the intent to award a Firm Fixed Price (FFP) IDIQ contract, utilizing procedures under FAR Part 15. The proposed contract action is to provide non-personal professional REGISTERED NURSING SERVICES FOR THE DESERT VISIONS YOUTH WELLNESS CENTER LOCATED IN SACATON, ARIZONA AND NEVADA SKIES YOUTH WELLNESS CENTER LOCATED IN WADSWORTH, NEVADA in accordance with the Scope of Work (SOW). Proposals are being requested and a written solicitation SF1449 is hereby issued. The Request for Proposal (RFP) number 75H71223R00052 is set-aside for Native American firms IAW Buy Indian Act of 1910 (14 MAR 22). This solicitation documents and incorporates provisions and clauses in effect through FAC 2023-04 effective 2 June 2023. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The North American Industry Classification System (NAICS) Code for this acquisition is 621399 for Offices of All Other Miscellaneous Health Practitioners with size standard of $10M; Product Service Code (PSC) is Q401 Medical- Nursing. Period of Performance Estimated base period of 21 August 2023 thru 20 August 2024 with four (4) option years Places of Performance: Desert Vision Youth Wellness Center 198 South Skill Center Road, P. O. Box 458 Sacaton, AZ 85247 AND Nevada Skies Youth Wellness Center 104 Big Bend Ranch Road, P.O. Box 280 Wadsworth, NV 89442 The Government will award a firm, fixed price contract resulting from this Solicitation to the responsive and responsible offeror whose conforming response is determined to provide the best value to the Government, price and other factors considered. The Government will only consider firm fixed-price proposals. Offeror to be considered responsible shall have an updated data and information in System for Award Management (SAM), FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items (NOV 2021).
