Statement of Work for Catholic Organist Chaplain Service The mission of Chaplain Service is to provide spiritual support and a pastoral care presence (24/7) for the patient's, visitors and staff of the Erie VA Medical Center. Our goals are to reduce spiritual injuries and to encourage others to draw from their own spiritual resources to enhance their quality of life. The Organist will provide organ or piano accompaniment for Chapel Services and special events. The Organist will have experience with either a VA chapel or a religious congregation. The Organist will provide a history of his/her music experience and provide references if requested. The Organist will possess the skills to perform music that is appropriate to a Roman Catholic Mass. The Organist will work with the assigned priest to coordinate the musical selections that will be incorporated into the service. LOCATION: VA Medical Center, 135 East 38th St., Erie, PA 16504 TIME AND FREQUENCY OF DUTIES: The current time and location of the Saturday Roman Catholic Mass is: Saturday evening, 2nd floor chapel 7 PM The Organist is expected to arrive ten minutes prior to any scheduled service or special event. Any changes in the time of the weekly service will be coordinated with the Organist. If the Organist will be unavailable for a service, the chaplain who will be leading the service should be notified as soon as possible. The Organist can designate a substitute for worship services that they cannot lead, but he or she is not responsible to have a substitute available for all services. Special programs will be announced as needed and in coordination with the schedule of the Organist. TRAINING All contractor employees who will be on-site at Erie VAMC are required to complete the following prior to starting work at the Erie VAMC: VA Privacy and Information Security Awareness and Rules of Behavior course and annually complete required security training; Failure to complete the mandatory annual training and sign the Rules of Behavior annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the contract until such time as the training and documents are complete. The contractor shall provide to the VA COR a copy of the completed training certificate(s) on an annual basis. PAYMENTS: Contractor will submit a monthly invoice, within 10 days after the end of each month, to the Contracting Officer s Representative (COR) for this contract. Invoices received without proper documentation as described below will be returned to the Contractor without action and payment will be delayed or disputed until proper invoicing is provided. Invoice for payment shall include the following: Contract Number Purchase/Delivery Order Number Invoice Date Dates of Service PERIOD OF PERFORMANCE: The government intends to award this contract for a six (6) month base period of performance with one 6-month option period. Base Period of Performance: 10/01/2018 to 03/31/2019 Option 1 Period of Performance: 04/01/2019 to 09/30/2019
